If you have claims issues or questions about any of your policies, please follow the following procedure.

Claim or Questions Procedure

  1. CONTACT THE INSURANCE COMPANY

    Contact the insurance company directly by using the attached contact information, or the phone number on the back of your insurance card. They will ask you for your identification number on the front of the card to insure accuracy. The insurance company is the best resource for quick resolution. If you lose your HealthScope card you can request a new card, and print temporary cards by logging into the HealthScope website at https://connect.healthaxis.com/hsb.aspx. For any other company cards or policies contact The Corley Agency.

  1. CONTACT The Corley Agency

    In the event that you do not receive a satisfactory solution, the next step is to contact The Corley Agency for all insurance needs or questions. They will assist you in finding a solution. They can be reached by telephone at (877) 506-3959.

    401K questions and enrollment will be processed by Schwab. Contact them toll free at (800) 724-7526 or visit their website. www.schwabplan.com

  1. Contact Amy Rowan in The Benefits Department if the need arises for major issues.
