All full-time associates are eligible to receive life and accidental death and dismemberment insurance. There is no cost to you to participate in this program. Please refer to the Summary Plan Description for more information.
In addition to your company-paid basic life insurance, you may choose to purchase supplemental life insurance through Lincoln for yourself, your spouse, and your child/ren up to age 18 (or up to age 25 if a full-time student). The premium for you and your spouse’s coverage is based on age and the amount of coverage you elect to purchase. Child life insurance may be purchased in flat amounts of $2,000, $5,000, $10,000, or $20,000 of coverage.. You may purchase supplemental life insurance for yourself in increments of $10,000 and in increments of $5,000 for your spouse, at a minimum of $10,000 coverage amount for each. In order to purchase life insurance for your spouse and/or child/ren, you must first purchase supplemental life insurance coverage for yourself. There is a guarantee coverage issue of $250,000 for supplemental employee life insurance and $25,000 for spouse life. Coverage in excess of the guarantee issue is subject to Evidence of Insurability (EOI) by providing a statement of good health to Lincoln.
Lincoln Voluntary Life and AD&D rates
Benefit enrollment and beneficiary forms should be sent to Amy Rowan in the Benefits Department. Your Evidence of Insurability form should be sent directly to Lincoln as instructed on the form.
Transamerica Term Level Life Insurance is a portable life insurance plan with a rate guarantee for twenty (20) years as long as premium is paid. Premiums are based on you and your spouse’s age and tobacco use status. Coverage is available for you, your spouse, and your child/ren under age 18 (up to age 25 if a full-time student).
