Eligibility

All Metromont associates who are regularly-scheduled to work a minimum of thirty (30) hours
per week are eligible for enrollment in the medical, dental vision, life, and disability insurance
plans. Coverage is available for you, your lawful spouse; your child(ren) up to age 26 ; or your disabled child of any age provided they were deemed disabled at the time of initial eligibility or at the time their coverage would otherwise have ended.

Enrollment

When you are eligible to enroll you will need the following information:

• Names, address, Social Security Numbers, and dates of birth of your eligible dependents
• Life insurance beneficiary information
• If you are enrolling your eligible dependents, please provide a marriage license for you and your spouse. Also, you will need to provide a birth certificate for your children; copies are acceptable.
